Understanding and Evaluating Health Technologies for Patients with Chronic Multiple Conditions

Digital health technology has the potential to support and improve disease self-management, care coordination, and medication management for patients with multiple chronic conditions. This project will produce a research report that provides criteria by which to evaluate and highlight promising applications and the measures relevant to identifying tools. The project will also address how these tools can be evaluated and integrated successfully for clinical decision making and disease management.
